Instructions for filing the short form

Step 1
Print a copy of the short form for this application.
STEP 2:
PROVIDE EROSION CONTROL INFORMATION IN THE FORM OF AN ATTACHMENT
WHEN BUILDING of STRUCTURES will disturb soil
NOTE: Wisconsin law required erosion control on all construction sites. Soil must not be allowed to be carried off your property by water runoff. If you are disturbing more than one acre you are required to file a storm water control plan with the DNR. Erosion enforcement could come from the county, the DNR or the State, depending on the sensitivity and size of the building area. If there are potential erosion control issues such as grades over 12%, they must be indicated on a BUILDING SITE PLAN and filed with this application. Show any wet lands, or water of any kind. Use arrows to show the downward direction of slopes. Remember that most erosion control inspectors generally believe that any ditch that flows to any protected watershed will need special attention including ditches along public roads.
It is the owners responsibility to comply with any county or DNR rules that apply to erosion, storm water and set-backs.
BUILDING MATERIAL WASTES AND LEAD PAINT
All building material wastes shall be managed to prevent pollutants and debris from being carried off the site by runoff. Waste is not to be buried or burned on site. Cement trucks must not wash up on streets or right of way. Owners could be charged for cement removal or repairs to roads in the public right of way.
Homes built prior to 1978 could contain lead paint which is dangerous to your health and especially for children. The Wisconsin Department of Health has issued rules that are to be followed when doing renovations in these structures. See the department WEB site: http://dhs.wisconsin.gov/lead/index.htm
SPECIAL DRAINAGE CONCERNS
Some building sites have potentially problematic drainage issues. It is the responsibility of the building permit applicant to consider soil and surface drainage conditions when placing any structure on a site. This includes how the placement will influence drainage from and to adjacent property. It is the designer and owners responsibility to provide methods to effectively control water ingress into a basement area and especially at widows used for egress from basements
COUNTY LAND USE PERMIT generally REQUIRED
You will often NEED A COUNTY LAND USE / ZONING PERMIT for projects that disturb soil in Oconto county. Call your county officials if your project significantly changes the building or disturbs soil for construction to check if you need a county permit.

STEP 3:
PROVIDE CONSTRUCTION INFORMATION IN THE FORM OF AN ATTACHMENT FOR BUILDING PROJECTS, REMODELING OR ADDITIONS WITH YOUR APPLICATION USING ANY ONE OF THE FOLLOWING METHODS:
(1) A set of building plans showing foundation requirements, intended use, building dimensions, window sizes, door sizes and general construction information needs to be sent with the application.
(2) A copy of a contract or a booklet from a manufacturer describing the building method and materials that will be used.
(3) A drawing or description developed by the applicant that provides the general constriction materials and methods that will be used.
STEP 4.
ENCLOSE CHECK FOR PREMIT FEE - Make check payable to TOM SMITH INSPECTIONS LLC.
Accessory building, shed, barn or detached garage: Base permit fees are for buildings that have minimal erosion impact, and are used for vehicle or similar storage but are not used for human habitation. SPECIAL NOTE: For any accessory building that is intended to be used as a sleeping area, have fireplaces, other HVAC equipment, plumbing or intended use that requires additional safety related inspection an additional .10 per square foot will be added to the base fee.
Accessory Buildings under 600 sq feet --------------NO permit required
Accessory Buildings 601 to 750 square feet---------$ 60
Accessory Buildings over 750 square feet-----------$ 90
Dwelling additions $100 plus 20 cents for each square foot over 600
STEP 5.
MAIL APPLICATION FORM, THE FEE PAYMENT AND SUPPORTING DOCUMENTS TO THE FOLLOWING ADDRESS.

Site work may start on the day you place the application in the mail.
Step 6: Your permit card will be returned to you by mail. Post in clear view of the street upon receipt.
CONTRACTORS ARE EXPECTED TO CALL FOR INSPECTIONS AS NEEDED FOR THE PROJECT. Generally an inspection is done for structures for the FOUNDATION prior to back fill. Followed by ELECTRICAL, PLUMBING or INSULATION if work is being covered and can’t be inspected as part of a FINAL INSPECTION when project is complete. Underground pools are inspected prior to the concrete shell being installed.
